CLINIC POLICY

  • Appointment scheduling: Patients may schedule appointments via phone, email or online portal for podiatric consultations, evaluations, treatments, and follow-up visits. If you need to cancel or reschedule your appointment, we require a minimum of 24 hours notice as a courtesy. Cancellations can be made by calling our office at 602.456.2085.

  • No-show & cancellation policy: Patients who do not provide at least 24 hours notice for cancellations or do not show up for a scheduled appointment will result in a $75 fee. This fee is not covered by insurance and is the patient's responsibility. Foothills Podiatry values your time, we ask that you be considerate of our schedule as well.

  • Late policy: Patients may be denied care if more than 15 minutes late to their appointment.

  • Patient check-in: Please plan to arrive 15 minutes before your appointment to fill out your new patient form. Upon arrival to the clinic please be ready to provide identification, insurance information, and any relevant medical history.

  • Clinic holiday schedule: Foothills Podiatry will be closed on all Federal holidays

  • Insurance and payment policies: Foothills Podiatry will perform it’s due diligence to verify insurance coverage prior to your visit however It is ultimately the patient’s responsibility to verify their own insurance eligibility and benefits. Please note that insurance is not a guarantee of coverage and that any balance not paid by insurance is the patient’s responsibility.

  • Work related forms: Patients who wish to have a work form completed must set up an appointment to review time off or work restrictions.

  • Limited space: We strive to make our patient visits as comfortable and accommodating as possible. Please be aware that our patient rooms can comfortably accommodate two people. Additional guests may remain in the clinic lobby during the visit.

  • Referral policy: If applicable, new patients should obtain podiatry referrals from their primary care physicians prior to scheduling an appointment to ensure medical insurance coverage. If you are on an AHCCCS or HMO health plan, please obtain a PCP referral before making an appointment.

  • Treatment consent: Informed consent from the patient, guardian or durable power of attorney will be requested prior to initiating any in-clinic procedures or surgeries.

  • Orthotics and durable medical equipment: The office staff will disclose CMS medicare DME MAC supplier standards and educate patients on proper medical device use. Patients will be required to sign a proof of delivery/dispense form upon receiving DME.

  • Surgery scheduling and preparation: Patients will be dispensed information on surgical scheduling, peri-operative expectations and post-operative care instructions.

  • Patient education: Upon checkout, if not already provided, patients will receive printed resources and materials to educate them about foot care, treatment options and preventive measures.

  • Infection control and sterilization: Foothills Podiatry maintains a clean and sterile environment, particularly in areas where invasive procedures are performed in compliance with Occupational Safety and Health Administration (OSHA) standards.

  • HIPAA compliance: Foothills Podiatry maintains compliance with patient privacy laws, including the protection of patient health information and maintaining confidentiality.

  • Continuing education and training: Providers and clinic staff are required stay current with developments in podiatric medicine and best practices.
